Volunteer Forms

Greater Fox Cities Area Habitat for Humanity requires that every volunteer have the following forms attached to each other on file at each worksite. 

• Adults over age 18
  • Worksite Registration Form
  • Release and Waiver of Liability Form
• Minors 14-17
  • Worksite Registration Form
  • Release and Waiver of Liabilty Form for Minors (Requires signature from guardian).
  • Age Restriction Policy

Starting August 1, 2009, every volunteer will need to have the above forms on file at every worksite where you volunteer. 

Fill each of these 2 forms out completely and sign them (you can photocopy it here in our office if you’d like) and bring one of each of these 2 forms attached to each other with you to all worksites where you volunteer with Habitat.
